Build your SNF online reputation with post-discharge review requests

How can a skilled nursing facility automatically generate review request drafts for recently discharged residents?
Every Monday, WebRun opens PointClickCare and finds residents discharged to home in the past seven days. It logs each with contact details in a Google Sheet and drafts a warm, brief review-request message in Gmail for the social worker to personalize and approve. No message reaches a former resident or family without a staff member reviewing it first.
- Every successful home discharge generates a review-request draft within the same week
- Social workers personalize a ready-made draft rather than composing outreach from scratch
- Only home discharges are included, excluding hospital transfers and SNF-to-SNF moves
Built for SNF social workers · skilled nursing marketing directors · long-term care administrators · SNF quality improvement teams

Every action is checked against this policy before it runs.
Questions, answered
Will it send review requests to former residents or families without staff approval?
No. Every message is drafted in Gmail and left unsent until the social worker reviews, personalizes, and approves it. No communication leaves the facility without human sign-off, ensuring the message is appropriate for each family.
Does the draft mention the resident's clinical stay or diagnosis?
No. The default draft thanks the family for choosing the facility and asks if they would share their experience. Clinical details, diagnoses, and stay length are not included. The social worker adds any personal context before approving.
What about residents who were transferred to another facility rather than going home?
It only drafts outreach for residents whose discharge destination is coded as Home or Community in PointClickCare. Transfers to hospitals, other SNFs, or hospice are excluded automatically.
