How does it know what the review is about?

It matches each review to the order behind it and the menu items on that order, so the drafted reply names the actual dish rather than answering in generalities.

What happens when the same item keeps getting complaints?

The existing Trello card is updated rather than duplicated, so a second complaint about the same item in one week shows as a pattern instead of two separate notes.
