Does it change anything in Act!?

No. It reads opportunities, history, and activities. It never edits a stage, closes a deal, logs an activity, or deletes a record in your database.

What counts as stale?

Any open opportunity with no activity or history logged for longer than the number of days you set, for example twenty one. Deals worked last week never reach the list.
