Will it approve or reply to comments without me?

No. WebRun only reads the held queue and drafts suggested replies into a Gmail draft. Approving, hiding, deleting, or posting a reply on your channel stays a manual click by you in YouTube Studio.

How does it tell a real question from spam?

It reads the comment text and the video it sits on, then sorts into genuine questions, ordinary praise, and obvious link bait. The spam tab is reported but never actioned, so a wrong call costs you nothing.

Does it touch comments that are already published?

No. WebRun works only the held for review and likely spam tabs. Comments already live on your videos are left alone entirely.
