Does it change or delete any screenshots?

No. It only reads what was shared and reports on it. Nothing in Droplr is edited or removed.

How does it know a share needs action?

It looks for a caption or annotation the sharer marked as needing action, and creates a Trello card only for those.

Will every screenshot get a Trello card?

No. Most shares just appear in the daily Teams digest. Only ones flagged as needing action get a follow-up card.
