How does it decide who needs a nudge?

It checks Stripe for a remaining drip credit balance, then checks PracticeQ for anyone who has not booked in 30 days, so only clients actually sitting on unused credit get flagged.

What if a client already used their credit?

It reads the live balance in Stripe each Monday, so anyone who booked and used their credit since the last run is left off the list.
