Automated WellSky Hospice Medication Reconciliation Queue

Every morning, WebRun opens WellSky, checks each active patient's medication reconciliation date and flags any recent order additions or changes, then posts a prioritized reconciliation queue to the hospice clinical Slack channel so nurses can complete or verify reconciliation before the day's visits.

In short

How do I automatically build a daily hospice medication reconciliation worklist?

WebRun opens WellSky each morning, checks every active patient for a new or changed medication order in the last 24 hours and for reconciliation dates that have lapsed, then posts a prioritized queue to the hospice clinical Slack channel. Clinicians start the day knowing exactly which patients need medication reconciliation before visits.

  • No patient starts a visit cycle with an unreconciled medication change
  • Clinical team has a ranked queue in Slack before morning visits
  • PHI stays in WellSky; only identifiers and flag reasons appear in Slack

Built for hospice nurses · hospice clinical directors · hospice pharmacists · palliative care teams

Good to know

Questions, answered

Does it send medication information to anyone outside the organization?

No. WebRun posts only patient identifiers and flag reasons to Slack, never medication names or dosages. All full medication details stay in WellSky, accessible only to authorized clinical staff.

What triggers a patient to appear on the queue?

Two triggers: a new or changed medication order in the last 24 hours, or a reconciliation date that has exceeded the configured window. Both are read directly from WellSky.

Can it reconcile medications on its own?

No. WebRun only builds the worklist. Medication reconciliation requires a licensed clinician to review the list in WellSky and confirm it is accurate.
