Will it fix a weak or breached password automatically?

No. WebRun only reports what needs attention. Changing or rotating a credential is left for whoever owns that vault item to do themselves.

How does it know an item is breached?

It reads the breach flag Enpass's own health check already assigns to the item, based on known compromised credential databases.
