Automated NexHealth Medication Titration Follow-Up Tracking

Every Monday, WebRun opens NexHealth and identifies endocrine patients who had a medication change (insulin dose adjustment, thyroid dose titration, or metformin start) and whose titration follow-up visit or lab check is now due but not yet scheduled. It posts a grouped worklist to the clinical Slack channel for the provider team to action.

In short

How can I automatically track which endocrine patients need a medication titration follow-up?

Every Monday, WebRun opens NexHealth and finds endocrine patients who had a medication change in the past 30 to 90 days and whose titration follow-up has not yet been scheduled. It groups the worklist by provider and posts it to the clinical Slack channel so every insulin, thyroid, or diabetes medication adjustment gets the timely review it requires.

  • No medication titration follow-up falls through the cracks each week
  • Providers start Monday with a grouped list of their own patients to action
  • Patients with both a lab order and an appointment are excluded automatically

Questions, answered

Will it contact patients directly to schedule their titration follow-up?

No. WebRun posts an internal worklist to Slack for the provider team to action. Any scheduling call or message to the patient is initiated by clinical or front-desk staff after reviewing the list.

Which medication changes trigger a follow-up on the worklist?

WebRun looks for documented medication changes in NexHealth visit notes including insulin dose adjustments, thyroid hormone titrations, metformin starts or dose changes, and GLP-1 dose escalations. The scope can be tuned to match your practice's protocols.

What if a patient has a lab order placed but no appointment scheduled?

If a lab order is in place, WebRun can identify it in NexHealth and mark the follow-up as partially addressed, flagging only those patients who have neither an appointment nor a lab order so the provider team sees the right gap.

Put this on autopilot.

Turn it on in minutes - or have our team set it up for you.
