Does the escalation text send itself?

No. The alert is queued in Twilio and held until a human approves and releases it, so no message goes to the duty manager without someone reading it first.

What counts as an escalating thread?

A mention you have marked negative that also passes the reply count you set, for example ten replies in a day, so ordinary grumbles stay in the digest and real pile-ons get flagged.
