How old does a pair need to be to get flagged?

Past 60 days unsold by default. That threshold is what separates normal shelf time from inventory that is genuinely dragging on your cash flow and margin.

Does it look at resale value or just what I paid?

Both. It pulls your cost basis from QuickBooks and compares it against the pair's current list price, so you see aging stock and overstated value together, not just one or the other.
