Automated Stroke Follow-Up Reminders

Every morning, WebRun opens Nexus Clinical, finds stroke patients discharged in the last 30 days who are due a follow-up visit, drafts a Twilio check-in text for staff to review and send asking how they are doing and whether they have scheduled their follow-up, and posts the stroke program coordinator a Slack list of who still needs to be booked.

In short

How do I make sure discharged stroke patients get their follow-up visit scheduled?

WebRun checks Nexus Clinical every morning for stroke patients discharged in the last 30 days who are due a follow-up visit with nothing booked. It drafts a Twilio check-in text for staff to approve and send, then posts the stroke program coordinator a Slack list ranked by urgency, so no discharged patient goes without a scheduled follow-up.

  • Every stroke discharge from the last 30 days is checked against its follow-up window
  • Coordinators see an urgency-ranked list every morning
  • Patients who already booked their own follow-up are removed automatically

Questions, answered

Will it text discharged patients without staff approving first?

No. WebRun only drafts the check-in text in Twilio and queues it for staff review. It sends only after a staff member approves it.

How does it decide which patients are overdue for a follow-up?

It compares each patient's clinically recommended follow-up window against what is actually booked in Nexus Clinical, so only patients with nothing scheduled in that window get flagged.

What if a patient already scheduled their own follow-up?

It reads the live scheduling status each morning, so a patient who already booked a visit is removed from the list automatically.
