How does it know a call is an emergency?

It looks for emergency keywords in voicemail transcripts and SMS messages - words like 'flood', 'burst', 'no heat', or 'urgent'. You can add or remove keywords from the list.

What if the on-call rota is not in Google Calendar?

You can point WebRun at any calendar or a simple spreadsheet. If no rota is found, it falls back to the manager's contact so nothing goes unalerted.

Can the technician acknowledge the alert from Telegram?

Telegram is the notify channel only - acknowledgement and job logging happen in your field service app. This keeps the alert simple and fast.
