Is this compliant with email marketing rules?

WebRun adds contacts with an opted-in source tag. You are responsible for confirming your list-building practices comply with CAN-SPAM, GDPR, or the rules in your jurisdiction before running this workflow.

What stops it adding people who already unsubscribed?

It checks existing audience status before adding anyone - unsubscribed and cleaned contacts are always skipped so your suppression list is respected.
