In short

How do I automatically flag dialysis patients at high risk of hospitalization each week?

WebRun opens Renvio every Monday, reads the predictive hospitalization-risk score for each active patient, and ranks the ones who are high risk or climbing fast. It posts a prioritized rounding list to your care team in Slack, showing each patient's risk band, weekly change, and top drivers, so the team can intervene before an admission.

  • Highest-risk patients surface every Monday before rounds
  • Care team sees why each patient's risk moved, not just a number
  • Rounding effort focuses on the patients most likely to be admitted

Questions, answered

Does any patient health information leave the clinic?

No. The risk digest is posted only inside your internal Slack workspace for the care team. Nothing is sent to patients or outside parties, so protected health information stays within your organization.

Where do the risk scores come from?

Straight from Renvio's predictive hospitalization-risk model. WebRun reads the score and drivers Renvio already calculates for each patient, ranks them, and hands your team a rounding list. It does not compute its own clinical scores.

Can it decide who gets an intervention?

No. WebRun only prioritizes and reports. Every clinical decision, such as who to call, adjust, or refer, stays with your interdisciplinary team, who use the ranked list as a starting point for rounds.
