Does the Teams post name patients by full name?

It uses whatever identifier your unit already shows on the internal census board, typically room number and last initial, and the post only goes to that unit's own charge nurse channel.

How does it decide which discharges are targeted for before noon?

It reads the estimated or physician confirmed discharge order already entered in Care Logistics for that day. Nothing is targeted unless a discharge order already exists.

What if a discharge order gets added or removed after the morning list goes out?

The 11:00 AM check and the 12:15 PM log both read live Care Logistics data, so a newly added or canceled discharge order is reflected without anyone updating the list by hand.
