How does it decide who counts as lapsed?

It uses the 60-day threshold by default and excludes guests with an active membership or an upcoming booking. You can adjust the threshold in WebRun's settings.

Can it vary the offer based on how many times a guest has visited?

Yes. Tell WebRun to use a higher-value offer for frequent guests and a lighter nudge for one-time visitors and it will apply that logic to every draft it creates.
