Will the referral go to the transplant center without review?

No. WebRun only drafts the referral packet in Gmail. The hepatologist reviews the clinical summary and sends it themselves.

How does it decide who is a transplant candidate?

It only acts on patients already flagged as a transplant candidate in gGastro (ModMed) by the treating hepatologist. It never makes that clinical judgment itself.

What happens if the transplant center does not confirm the evaluation date?

The candidate stays marked as pending in the Google Sheets tracker until a status update is entered, so nothing silently falls off the list.
