Automated Sigmund Aftercare Referral Tracking

Every morning, WebRun opens Sigmund Software, checks the aftercare referral status for every patient discharged in the last 30 days, logs where each referral stands to a tracker sheet, drafts a check in email to any receiving provider who hasn't confirmed the patient's intake within 7 days, and posts a Slack alert for referrals that have stalled.

In short

How do I make sure aftercare referrals don't stall after discharge?

WebRun checks Sigmund Software every morning for every patient discharged in the last 30 days, logging each aftercare referral's status to a tracker sheet. When a receiving provider hasn't confirmed intake within 7 days, it drafts a check in email in Gmail for the discharge planner to review and send, and posts a Slack alert naming every stalled referral to the clinical team.

  • Stalled aftercare referrals surface within 7 days instead of being discovered at a relapse readmission
  • The full referral pipeline is visible in one tracker sheet, not scattered discharge summaries
  • Provider check in emails are drafted and ready before the discharge planner has to chase anything

Questions, answered

Will it contact the aftercare provider without a discharge planner reviewing it first?

No. Every check in email is saved as a Gmail draft under the discharge planner's account. Nothing is sent to an outside provider until a human reviews and sends it.

How long does a referral get before it's flagged as stalled?

By default, 7 days without a confirmed intake, though the window can be adjusted in your run settings to match how your program defines a stalled referral.

What happens once intake is confirmed?

It reads the confirmed intake status in Sigmund Software each morning and marks that referral closed in the tracker sheet, so only genuinely open referrals stay on the active list.

Put this on autopilot.

Turn it on in minutes - or have our team set it up for you.
