Does it change who is on call?

No. It only mirrors the roster QGenda already has confirmed. Swapping or assigning who is on call is still done in QGenda by your scheduling office.

Who receives the Teams reminder?

Only the individual provider whose own shift starts within the next 24 hours. It never messages a provider about someone else's shift.

What happens if a shift gets swapped last minute?

The next morning's run reads the current roster fresh, so a swap made in QGenda is picked up and the calendar and reminders update automatically.
